Motor replacement

I recently moved into a house and soon discovered the pool pump motor needs replaced. Unfortunately, the label has partially eroded. I was hoping you might be able to help me decipher what motor replacement and shaft seal I would need from the available information. THANKS!!

The replacement motor for your Jandy PHPM1.0 is model B2853. The shaft seal is part number 5020-086.
